If you need health insurance, you can be enrolled in a comprehensive medical, dental, and vision insurance plan beginning the first day of regular employment. With health care costs on the rise, you'll be comforted to know that we're doing all we can to keep your premiums down and quality up.
Medical Plan Options: NetApp offers a choice of health insurance plans, so you can choose the balance of cost and access to care that best meets your needs. Each plan offers four tiers of coverage: yourself only, you plus your spouse or domestic partner, or you plus your child or children and you plus your family (spouse/domestic partner and child(ren).
CIGNA Traditional Plan: The Traditional Plan is a familiar, classic PPO. You have full coverage for preventive care in-network and pay for other expenses through copays, coinsurance and your deductible. The Traditional Plan provides in- and out-of-network coverage for health care services. You’ll save money when you use in-network providers.
CIGNA Health Savings Plan: The Health Savings Plan takes care of you today and offers you the unique advantage of helping you save for your future. This is a PPO plan with a high deductible that is paired with a tax-advantaged health savings account (HSA)—you can make your health dollars go further with tax savings, interest and investment earnings.
You have full coverage for preventive care in-network and pay for other expenses through your deductible and coinsurance. You’ll get coverage for in- and out-of-network health care with the Health Savings Plan. You pay a lower amount every paycheck and then manage more of the initial costs for care during the year—so you never pay for insurance that you don’t need.
Kaiser HMO: In addition to the Traditional and Health Savings Plans, NetApp offers a Kaiser HMO to employees located in California. With the Kaiser HMO, you have a copayment for most services. You do not have to meet a deductible or pay coinsurance. To receive benefits, however, you must use doctors and hospitals in the plan’s network (except in an emergency).

MetLife Dental Plan: The NetApp Dental Plan lets you see the dentist of your choice and enjoy the same benefit coverage anywhere, up to an annual maximum of $2,500.
Preventive treatment, including two checkups annually, is covered at 100%.
Basic treatment is covered at 80% after an annual deductible.
Major treatment, including orthodontia, is covered at 50% after an annual deductible. Orthodontia has a lifetime limit of $2,000 and is available for adults and children.
Keep in mind, however, that out-of-pocket expenses are significantly lower if you select a dentist within the MetLife network.
VSP Vision Plan: Benefits within the VSP network include an eye examination and prescription glasses (lenses and frames), covered in full after a $20 copay, each calendar year. Contact lenses can also be covered instead of glasses, up to $150 per year in network.
Wellness Programs: A wide range of wellness benefits are available to all employees, at no additional cost, through your medical plan provider.
For CIGNA members, the Well Aware programs offer focused comprehensive care for a variety of health issues. A dedicated staff of clinicians, educational resources, planning tools, health reminders, and a 24-hour nurse information line provide personalized support to help you monitor and manage your health more effectively.
Disease management programs include asthma, diabetes, heart disease, low back pain, COPD, weight management, and depression. The Targeted Conditions program covers many other common health conditions, from acid reflux to osteoarthritis.
For Kaiser members, a variety of resources are available to help you live a healthier life. These include extensive online resources and tools; member discounts on health and fitness options; and Healthy Lifestyle programs for issues such as weight loss, stress reduction, nutrition, and smoking cessation.
Adoption Assistance - To support our employees who pursue this route to parenthood, this program covers up to $12,650 (2012 IRS maximum) in adoption expenses per calendar year and $20,000 lifetime expenses per family.

Short-Term Disability (STD): When you can't work due to injury or illness, this employer-paid program through Liberty Mutual provides income protection for you and your family.
Non-California locations: STD pays two-thirds of your income, after a seven-day waiting period. You may receive up to $4,154 per week for up to 90 days.
California locations: You receive similar benefits under the California Voluntary Disability Insurance (VDI) plan. VDI begins on the 8th day of disability and pays two-thirds of your income. You may receive up to $4,154 per week for up to 52 weeks following the date of disability.
Long-Term Disability (LTD): LTD provides extended peace of mind, starting coverage when STD ends after the 90th day of disability. You can choose from two employer-paid options through Liberty Mutual, one of which offers a tax-free benefit:
Option 1 provides two-thirds of your monthly earnings if you are disabled. Any benefits you may receive are taxed as regular income.
Option 2 provides 60% of your earnings tax-free. Because the premium contributions NetApp pays on your behalf are taxable, the benefits are not taxable. This option generally results in a higher benefit than Option 1.
Basic Life and AD&D Insurance: Basic Life and AD&D insurance are provided through MetLife at no cost to you. Both programs cover twice your annual earnings up to a maximum of $1.5 million.
Supplemental Life Insurance: Optional coverage is also available though MetLife, for a benefit of up to eight times your salary up to a maximum of $3 million, subject to evidence of insurability (EOI). Coverage is also available for your spouse or domestic partner up to $250,000, subject to EOI, and at $10,000 for your children.

Flexible Spending Accounts (FSAs): NetApp offers you a variety of flexible spending accounts (FSAs) to help with qualified out-of-pocket expenses throughout the year. You can contribute pre-tax dollars to the accounts, administered through PayFlex, to help cover qualified health care, day care, mass transit and/or parking expenses. You decide how much to contribute (up to IRS limits), based on your family’s needs. Your contributions are deducted from your paycheck before taxes—saving you money on items you’d need to pay for anyway. Carefully estimate your contributions, because any money left at the end of the year will be forfeited. You can use any or all of these accounts:
Health Care FSA: his account helps you pay for qualified out-of-pocket expenses not covered by the health plans. To make things even easier, you pay for services directly out of your FSA with a convenient debit card. You do not need to be enrolled in a NetApp medical plan to contribute to the health care FSA. The 2012 maximum contribution is $5,000.
Limited Purpose FSA: If you are contributing to a health savings account (HSA), you are ineligible to enroll in NetApp’s “general purpose” health care FSA, but may participate in the “limited purpose” FSA to pay for qualified dental and vision expenses. The annual contribution limit for this plan is $2,500. Depending on your planned expenses, the “limited purpose” health care FSA may be attractive because it allows you to reserve your HSA balance for medical expenses only and to save HSA funds for the future. Keep in mind that the FSA funds are “use it or lose it” and do not roll over year to year.
Day Care FSA This account helps you pay for daytime care for those who depend on you while you’re at work. Whether you’ve got children, elderly parents or a disabled spouse who resides with you and requires care, this account assists with those qualified expenses. You can enroll in this account and select your contribution every year only during open enrollment (unless you experience a qualified life event such as the birth or adoption of a new child). The IRS sets the maximum contribution, which is $5,000 per year.
Transit/Parking FSA: This FSA helps you save on qualified expenses related to your commute to work. You can use your contributions to pay for things such as parking, van pooling and mass-transit tickets. The maximum contribution is determined each year by the IRS.
401(k) Plan: The NetApp 401(k) plan features a 100% match on the first $1,000 you contribute each year, then matches 50% up to a total maximum match of $3,000 per calendar year (with a three-year vesting period). A broad range of investment options are available from Fidelity Investments to suit your needs and investment style. A "catch-up option" is also available for employees who are age 50 and over.
Employee Stock Purchase: The Employee Stock Purchase Plan (ESPP) allows all regular employees who work more than 20 hours per week to purchase NetApp stock through accumulated payroll deductions. Eligible employees can enroll twice each year, into a 24-month Offering Period beginning June 1 or December 1. Purchase periods are every 6 months, and the purchase price is equal to 85% of the lower of (1) the closing price of the stock on your enrollment date into the Offering Period, or (2) the closing price of the stock on the purchase date.

Time Off: Paid vacation time increases based on years of service. You accrue 12 days per year in your first two years, 15 days from year two through year six, 20 days from year six through 10, and 25 days after more than 10 years. Time off also includes 10 paid holidays and sick time.
Volunteer Time Off (VTO) supports employees who choose to serve their communities, offering up to five days a year with full pay for time off spent performing volunteer work.
Employee Assistance Program (EAP): Employees can get free, confidential assistance with issues such as individual or family counseling, work/life conflicts, and legal and financial services. Services are provided through CONCERN.
Educational Assistance: The Tuition Reimbursement Plan, created as a result of employee requests, covers expenses of up to $5,250 per year for accredited coursework. NetAppU also offers a wide range of free professional development resources.
Prepaid Legal Plan: The Hyatt Legal Plan provides covered legal services, consultations on the telephone, in-person consultations, document preparation, and representation in many frequently needed legal matters.
